I certify that I am the parent or legal guardian for my child(ren). I hereby waive, release and forever discharge Harbor View Swim Team (HVST), its directors, officers, agents, coaches, volunteers, sponsors and employees from liability from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Harbor View Swim Team (HVST) activities, whether or not damages or loss is due to negligence. 

I hereby acknowledge that my children is (are) physically fit and capable of participation in all Swim Team activities.

In consideration of the acceptance of this application and registration, I the undersigned parent or legal guardian, having legal custody of or authority for the child/children for whom this application and registration is submitted, and intending to be legally bound, do hereby for the child/children, the child's parent or guardians and minor siblings, heirs, executors and administrators, waiver, release and forever discharge any and all rights and claims for damages which any of these persons may hereafter have against Harbor View Swim Team (HVST), the Harbor View Community Association, the South Coast Swim Conference, or any subcommittee, representative, agent, or assigned for any and all injuries which may be sustained in transportation to and from or participation in the activities of the Swim Team, including presence as a spectator.

By registering my child(ren) with Harbor View Swim Team (HVST), I agree to participate (or allow my child(ren) and family members to participate) in Harbor View Swim Team (HVST), and hereby release Harbor View Swim Team (HVST), its directors, officers, agents, coaches, volunteers, sponsors and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Harbor View Swim Team (HVST) program, including travel to and from training sessions, swim meets or other scheduled team activities.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participate in the Harbor View Swim Team (HVST) program.

The child/children named in this application and registration is/are in good physical condition and can participate in all of the activities of the Harbor View Swim Team (HVST). I assume complete responsibility for notification of the officers and coaching staff of the Swim Team regarding ANY physical, medical or emotional condition which may affect the child/children in relation to Swim Team activities, which notification will be delivered in writing. In the event that the child/children is/are provided emergency medical care under circumstances where I cannot be immediately notified, I agree to reimburse any and all expenditures for this care. HVST's Board of Directors and/or coaches reserve the right to deny membership or discontinue a swimmer's association with the team without refund as necessary for the best interest of the team.

State law requires youth athletes and their guardians receive a copy of the Concussion Information Sheet in order to participate in athletics. By signing this electronically, you acknowledge that you have received the Factsheet and will review the contents with the participating swimmer.

Harbor View Swim Team (HVST) is a volunteer driven swim team. It takes nearly 100 volunteers to run a full swim meet and cannot happen without your help. In order for your swimmer(s) to swim on Harbor View Swim Team (HVST), you are required to sign up for four volunteer shifts (Aquaman, dual home meets, Swim-a-plooza and minimum one away meet). A volunteer shift is one-half of one meet. All positions are subject to change.

* If you are ONLY registering swimmer(s) that are 13 years old or older, then you are exempt from volunteer requirements.

* Note that additional volunteering may be required if your child is participating in SCSC Relay Finals and/or SCSC Championships.

* Please also note, if you no-show the day of the shift without someone covering your shift, you will automatically get charged $125 for that shift.

By signing this agreement, you are agreeing to complete your volunteer shifts. If you are unable to fulfil your required shift, you are responsible for finding a replacement and/or you will assign yourself another shift for a different meet.

At the end of the season, if a family's volunteer commitment is not met, the credit card on file will be charged $125 for each missed shift for a maximum of $500. Please note that you will see a conditional pre-approval for this charge at checkout; however, rest-assured that you will not be charged if volunteer commitments are fulfilled.

If you wish to wholly withdraw from the team, you need to issue a formal written request to the harbor view email account by May 11th (5/11/25) to be eligible, where the board will then review each case. There will not be any registration refunds given by this request but it would be a decision of waiving the volunteer requirement or not.

As part of the Harbor View Swim Team (HVST) communication and promotional process, HVST maintains a website, posts social media content, prints materials, and advertises in print publications. The team may also allow members of the media to take pictures of swimmers, swim events, and other team activities for publication. The team feels these are legitimate opportunities for swimmers to be in the spotlight and also promote the team. By selecting yes, I hereby authorize HVST to publish the photographs, videos, and images taken of me and/or the minor children that I have registered as swimmers for HVST in print publications, online publications, presentations, websites, and social media. I also understand and agree that no royalty, fee or other compensation shall become payable to me or the minor children by reason of such use. I further agree that participation in any publication, website, or other media produced or posted by HVST confers no rights of ownership whatsoever. By completing this form, you consent to your photographs to be used as described above.

If you do not live in Phase I of Harbor View (HVCA), and you are registering a swimmer under 13 years old, an out of area fee of $50 per swimmer will be added to your payment. For the avoidance of doubt, this $50 fee per swimmer must be paid by household addresses in Phase II of Harbor View Homes (NHCA) and Phase III of Harbor View Homes (Seawind).

Registration will not be processed without the correct fee applied. If you fail to pay the out of area fee and live outside of HVCA, your swimmer’s registration will be placed on hold.

Harbor View Swim Team (HVST) is a sanctioned program located at the Harbor View Community Pool, 1854 Port Westbourne Place, Newport Beach, CA. The Harbor View Community Pool is a controlled access facility. This facility is for use by the HVCA (phase 1) residents and swimmers enrolled in the 2025 season of Harbor View swim Team ONLY.

Non-residents participating in or attending a sanctioned program or event held by HVST must vacate the facility once the program/event has concluded. Unauthorized persons present are trespassing and will be asked to leave the facility.

Harbor View Swim Team (HVST) enrolled members are seasonal only, beginning the first day of swim practice on April 14th to July 18th, 2025, the last day of swim practice.

Family members and friends of an enrolled HVST swimmer are NOT permitted to use the Harbor View Community pool or small pool. Non-members are however allowed on the pool deck only for visitation during the enrolled swimmers practice hours and shall leave immediately after.

I hereby acknowledge that all non HVST enrolled members will not use the Harbor View Community pool as stated above. Violation of the above rules may result in suspension of participating on the Harbor View Swim Team and access to the facility as well as other enforced measures. Association, Management personnel and Coaches have authority to exclude an individual(s) from the facility.

Refund of registration fees will be honored through April 14, 2025 if requests are received to [email protected] less a $50 cancellation fee. After that date, no refunds will be given. There is no pro-rating of fees for partial attendance due to absences, injury or inclement weather.

If you have a 4 year old that you are interested in registering for the summer swim program, please reach out to one of the coaches first to schedule a tryout. Alternatively, you may email [email protected] to get connected to a coach to schedule. If you register without a coaches approval and need to cancel, then you may be subject to a cancellation fee.
